Computer-Aided Decision Support (CADSS) in AAP began in Leeds, in the Professorial Surgical Unit and the Accident & Emergency Dept, at the General Infirmary in the early 1970's. The instigator (visionary or madman, depending on your attitude to computers way back then) was Professor Tim de Dombal.

In the initial studies a computer system (based on a Bayesian analysis of patients' symptoms and signs) outperformed clinicians. However, the clinicians' own diagnostic accuracy improved whilst using the system. Decision making also improved, shown by a fall in the negative laparotomy and perforated appendicectomy rates (1,2,3).

In the late 1970's the system was transferred to an Accident and Emergency Dept, the database increased to include more disease sites, particularly gynaecology causes for AAP and over a 13 month study period similar results were found ...........
